Rules differ by nation. England's thresholds are the default.

Total savings and investments

Cash, bank balances, ISAs, premium bonds. Do not include property value here.

Owns a property

Including with a mortgage. Joint ownership counts.

Total weekly income

State pension, private pension, employment income, benefits.

Complex or significant health needs

e.g. advanced dementia, end-of-life care, complex post-stroke needs, severe Parkinson's. This flags whether NHS Continuing Healthcare (CHC) is worth investigating — CHC is needs-led, not means-tested.
